Qualifications: We are seeking a full-time, Bilingual (Spanish Speaking) Licensed Clinical Social Worker (LCSW)/Behavioral Health Specialist to expand our integrated behavioral health program. Working under the direction of the Behavioral Health Director as part of a team that includes medical providers, support staff, promatoras de salud, health educators, dietitians and othres, the LCSW will provide behavioral health services to patients at Winters Healthcare.

The City of Winters is located at the base of the Vaca Mountains in Northern California. Winters ia a small farming commuity, home to unique shops, restaurants, winteries and galleries. Winters offers its residents a host of amenitites, recreation activities, and world class cultural offerings both in town and within a short driving distance. The Pacific coast and the Sierra Mountain range are both within 100 miles and San Francisco is 75 miles away. Even closer to Winters are the Lake Berryessa recreational area and Napa Valley.
Responsibilities and Skills
1. Provides behavioral health screening for medical patients on an annual basis.
2. Meets with all adult patients on an annual basis for a "Lifestyle check-in".
3. Works with medical providers and clinic staff to provide behavioral health intervention during medical visits.
4. Meets with patients who are referred by providers for short-term behavioral health interventions for a variety of problems, pain control, behavioral problems of children, adolescents, adults.
5. Accumulates knowledge of community resources for support services such as transportation, food, housing, legal aid and ensurest hat the information is readily available to clients.
6. Keeps notes appropriate for behavioral health visits using electronic medical records.
7. Works closely with healthcare team to provide coordination of care and support of patients.
8. Works with Behavioral Health Director and other appropriate staff to provide groups pretinent to behavioral health and mental health such as caregiver support groups, parenting groups, etc.
9. Works with schools to coordinate services to children seen in behavioral health program when appropriate.
10.Assists with other projects as assigned.
